Diane was born in Houston, Texas, on October 11, 1955. She was graduated from Memorial High School and the University of Houston. Diane made her home in Portland, Oregon, for the past twenty years where she was credit manager for the Western Region of Adidas, Inc. Diane is preceded in death by her husband, Thomas Dewitt Forner, MD., and her father, Charles A. Pace, Jr. She is survived by her daughter, Robin Christa Forner, her mother, Lou Hardy Jax, her stepfather, James W. Jax, her sister, C. Renee Mitchell, her stepbrothers and sister, Clayton, Garth, Brady Jax and Hilary Jax Thomopoulos, her aunts and uncles, her sisters and brothers-in-law and several nieces and nephews. The family wishes to express their immense gratitude to Dr. Kevin Olson of Portland, Oregon, Drs. Katherine and Peter Pisters, the MD Anderson Cancer Center medical team, the Vitas Hospice, and her faithful caretakers, Evelyn, Miriam, Sonia, Lovie, Teresa, and Rose, all of Houston, for their individual support, loving care and medical expertise. A memorial service will be held at 3:00 p.m., Saturday, April 2, 2005, at St. Francis Episcopal Church, 345 Piney Point, Houston, Texas 77024. On April 9, 2005, following a memorial service at the Riverview Cemetery, Portland, Oregon, for her friends, neighbors and co-workers, Diane will be interred beside her beloved husband, Tom. In lieu of flowers, the family requests that donations be made to the Susan G. Komen Breast Cancer Foundation or the Oregon Donor Program.
