- John Horace Gay died on 28 June 1915 at age 1 in Pisgah, Woodford County, Kentucky, at the old James Gay farm, Pisgah Pike of acute colitis. . His death was officially witnessed by John Horace Gay.
- He was interred at Pisgah Church Cemetery, Versailles, Woodford County, Kentucky.
- The following appeared on 29 June 1915 in The Lexington Herald: John H. Gay, Jr., the eighteen-months-old son of Mr. and Mrs. John Horace Gay, died at the home of his parents, near Pisgah, this county, Monday morning at 5 o'clock of colitis, following an illness of several days. The funeral services will be held at the family residence Tuesday afternoon at 3 o'clock, the Rev. William T. Punch, of this city, officiating. Interment will be made in the Pisgah Cemetery. The pallbearers will be Claude Williams, James Cox, John T. Gay and R. H. Gay.
